Why Hangovers Feel Worse With Age — What the Research Shows
Quick answer: Large surveys find the opposite of the folk wisdom: hangover frequency and severity go down with age, not up. What rises with age is everything around the hangover — a higher blood alcohol from the same drink, sleep that recovers less well, medications in the mix, and a next day with more in it. The felt experience is real; the usual explanation for it is not.
Somewhere in your thirties the arithmetic changes. The same night out that used to cost a slow morning now costs a whole day, sometimes two. Nearly everyone reports it, and the standard explanations get repeated everywhere: your liver is slowing down, your enzymes are failing, your body can't process alcohol anymore.
The research says something more interesting, and more useful.
What the Surveys Actually Found
Three separate studies, three different methods, one direction.
The population survey. Tolstrup, Stephens and Grønbaek examined 51,645 men and women aged 18 to 94 in the Danish Health Examination Study. Compared with people aged 60 and over, the odds of severe hangover after binge drinking were 6.8, 4.8, 3.0 and 2.0 among men aged 18–29, 30–39, 40–49 and 50–59, with a similar pattern in women. Crucially, that gradient could not be explained by the usual amount of alcohol consumed, the frequency of binge drinking, or the proportion of alcohol consumed with meals. Their conclusion: hangover after binge drinking "is much more common in the young than in the older age groups."
The lifespan study. A survey of 761 Dutch drinkers aged 18 to 94 found significant negative correlations between age and hangover severity (r = −0.327, P < 0.0001) and between age and hangover frequency (r = −0.195, P < 0.0001). The authors suggest an age-related decline in pain sensitivity may partly explain the pattern, and note that sex differences in hangover were largest in the youngest groups and faded in the older ones.
The day-by-day study. Rather than asking people to remember, Huntley and colleagues used daily diaries from 274 drinkers aged 15 to 66 across 4 to 14 days and found hangover was more severe among younger drinkers, especially at higher drinking levels, even after controlling for each person's typical consumption.
So: three designs, no support for the idea that a given night of drinking produces a worse hangover at 45 than at 25. Which raises the actual question.
So Why Does It Feel Worse?
Here are the candidates, ranked by how well the evidence supports them. The first two are solid, the third is well documented, and the last two are plausible stories rather than findings — labeled as such, because the difference matters.
1. The same drink now produces a higher blood alcohol
This is the best-supported mechanism, and it is not about a failing liver. Alcohol distributes through body water, and water distribution volume decreases with age while the activity of the enzymes involved — alcohol dehydrogenase, acetaldehyde dehydrogenase and cytochrome P-4502E1 — diminishes. The review's summary of the result: both changes "lead to increased blood concentrations of ethanol," with central nervous system effects appearing at lower intakes because the aging brain and liver are more sensitive.
Read that carefully, because it reframes the whole question. Four drinks at 50 is a bigger dose than four drinks at 25 — not the same dose processed worse. And since hangover severity tracks how much alcohol reached your system, the honest version of "hangovers get worse with age" is closer to the same order gets you further along the scale than it used to. 2. Sleep does less repair than it used to
Alcohol wrecks sleep at any age: more deep N3 sleep early, less REM, then a rise in the lightest N1 stage once the alcohol is metabolized, producing frequent wakings and fragmented, low-quality sleep, with heart rate raised in the second half of the night.
The difference is the sleep it lands on. With age, people spend more time in the earlier, lighter stages and less in the deeper ones, waking more often and getting more fragmented, less restful sleep, plus a circadian shift toward earlier waking. Alcohol degrades a night that already has less margin — and there is no compensating drop in need, since the idea that older adults require less sleep is a misconception; the target is still at least seven hours.
Sleep length also tracks hangover severity directly. In a study of 578 students, total sleep time correlated negatively with hangover severity (r = −0.178) — shorter nights, worse mornings — while total alcohol consumed did not significantly predict severity in that dataset. 3. Medications are now in the mix
Midlife is when prescriptions accumulate, and alcohol interacts with a lot of them. A 2026 review found estimates of older adults concurrently using alcohol and potentially interacting medications ranging from about 20% to nearly 80%, averaging 25.1%, and lists excessive sedation with benzodiazepines, opioids or antipsychotics, enhanced hypoglycemia with some diabetes drugs, and raised bleeding risk with NSAIDs — which is worth knowing before the reflexive ibuprofen the next morning. The same review notes reduced liver blood flow and body-composition changes mean alcohol "tends to reach higher concentrations at the same dose."
4. The next day has more in it (plausible, not measured)
At 22 a hangover could be spent horizontal. At 42 it happens on top of work, children, and obligations that do not reschedule. The same symptoms produce far more disruption, which is not the same as being more severe — but it is what most people are describing when they say it got worse. Treat this as a reasonable explanation, not a finding; nobody has measured "hangover cost by life stage."
5. Recovery drifts slowly, so the comparison is against memory (plausible, not measured)
You are not comparing this morning to last month. You are comparing it to a version of yourself from twenty years ago, remembered generously. Slow changes across a decade get attributed to whichever night made them noticeable. Again: an honest story, not evidence.
What a Hangover Actually Is
The mechanism is less settled than the internet's confidence suggests, and "dehydration" is only a fraction of it.
A hangover develops as blood alcohol concentration approaches zero — which is why it arrives in the morning rather than at the peak of drinking. Reviewing the biology, Palmer and colleagues conclude that alcohol metabolites, neurotransmitter alterations, inflammatory factors and mitochondrial dysfunction "are the most likely factors in hangover pathology," having examined alcohol's metabolites, neurotransmitter changes with particular reference to glutamate, neuroinflammation and ingested congeners.
Four working parts, then, none of which a glass of water fixes:
- Acetaldehyde and the metabolic aftermath — alcohol's first breakdown product is toxic, and the clean-up produces oxidative stress.
- The glutamate rebound — alcohol suppresses the brain's excitatory system while present; the compensation is unmasked when it clears, which is the direct driver of the anxiety, jitteriness and noise sensitivity. That is the same mechanism behind hangxiety.

- Fragmented sleep — the thing you experience as exhaustion.
Fluid loss is real and contributes to thirst and headache. It is one input among several, which is why hydration blunts a hangover without preventing one.
How Long a Hangover Lasts
Better documented than most people expect. Measuring severity every two hours in 811 drinkers, researchers found the average duration of the alcohol hangover was 18.4 (3.8) hours, with the majority ranging from 14 to 23 hours from the last drink — roughly 12 hours from the time of waking.
That study was conducted in students aged 18 to 30, so read it as the shape of a typical hangover rather than a personal forecast. The 2-Day Hangover: What Is Actually Going On
A hangover that runs into a second day is outside the typical range, so it deserves an explanation rather than a shrug. Three of them account for most cases.
The clock started later than you think. Hangover timing is anchored to the moment blood alcohol reaches zero, not to when you went to bed. Drinking heavily until 3am can put that moment well into the following afternoon; an 18-hour hangover from there lands squarely in day two. It is a hangover plus a sleep debt. One badly fragmented night is bad. Two — the drinking night, then the night after, when the rebound keeps sleep light — produces a second day that feels like a hangover but is substantially sleep deprivation. This is why the second day often has more fog and low mood than headache and nausea.
It is not a hangover. This is the one to take seriously. Congeners: Why the Whiskey Reads Worse
One folk belief holds up. In a controlled study, 95 heavy drinkers drank to a mean of 0.11 g% breath alcohol on vodka or bourbon on separate nights — bourbon containing roughly 37 times the congeners of vodka. The result: no effect of beverage congeners was found except on hangover severity, with people feeling worse after bourbon.
Note the boundary. Alcohol itself decreased sleep efficiency and REM sleep, and increased wake time and next-day sleepiness, and next-day cognitive impairment was the same for both drinks. Darker spirits make the morning feel worse; they do not make you more impaired. Switching to clear spirits is a comfort adjustment, not a safety measure.
What Helps, and What Doesn't
The honest summary: nothing reliably treats a hangover once it exists. A systematic review of randomized placebo-controlled trials covering 21 studies and 386 participants concluded that "only very low quality evidence of efficacy is available to recommend any pharmacologically active intervention" for preventing or treating hangover. "Hair of the dog" is worth naming separately, because its mechanism is definitional. A hangover starts as blood alcohol approaches zero; adding alcohol pushes that moment further away. The relief is real and the hangover is not cancelled — it is rescheduled, at a larger dose. As a regular strategy it is also one of the patterns that most reliably escalates drinking.
What does work is unglamorous: less alcohol, more time between the last drink and bed, food, water alongside, and protecting the sleep. Every one of those reduces the size of the event rather than treating it afterward.
Use the Data, Not the Guilt
A bad morning is worth something if it produces a change to the plan rather than a round of self-criticism. Two things make that concrete.
